Services

Palliative care services are available to people living with any life limiting illness. This individual must give their consent to access services. We provide individualized care, which is guided by the unique needs and goals of each person served. These are identified through initial and ongoing exploration of what is important to the individual and their family. A care plan is jointly developed for the delivery of palliative care services. Individual centered care entails meeting people where they are at, (physically, emotionally, spiritually) to gain understanding from their perspective. Individual centered care affords respect, dignity and choice.
